Subway directions

  • From West Side: Take Brooklyn bound C train to 23rd Street. Depart 23rd Street station on 8th Avenue, heading North. Turn right onto 26th Street and see entrance on left between Duane Reade Pharmacy and Oakwood Restaurant.
  • From the East side: Take the Shuttle from Grand Central to Times Square. Take the Brooklyn Bound C train to 23rd Street. Depart 23rd Street station on 8th Avenue, heading North. Turn right onto 26th Street and see entrance on left between Duane Reade Pharmacy and Oakwood Restaurant.
  • From New Jersey: Take the Path Train. Exit at 23rd Street at 6th Avenue, walk West towards 8th Avenue. Turn right onto 8th Avenue, to 26th Street. Turn right onto 26th Street and see entrance on left between Duane Reade Pharmacy and Oakwood Restaurant.
  • From Staten Island: Take the Staten Island Ferry to South Ferry. Take the 1 train to Chambers Street, transfer to the C train. Take the Uptown C train to 23rd Street. Depart 23rd Street station on 8th Avenue, heading North. Turn right onto 26th Street and see entrance on left between Duane Reade Pharmacy and Oakwood Restaurant.

Driving directions

  • From Staten Island: Go over the Verrazzano Bridge. Follow BQE toward Manhattan. Exit at Brooklyn Battery Tunnel, go through tunnel. Follow signs to West Side Highway. Take West Side Highway to 23th Street. Turn right onto 23th Street. Continue East to 8th Avenue. Turn left on 8th Avenue. Continue on 8th Avenue to 26th Street. Turn right onto 8th Avenue and see parking on your right. Hazelden entrance is located across the street between Duane Reade Pharmacy and the Oakwood Restaurant.
  • From Long Island: Follow the Grand Central Parkway or the Long Island Expressway to the Midtown Tunnel. Turn right onto East 34th Street. Turn left onto 9th Ave. Turn left onto West 26th. Continue East to 8th Avenue, cross over 8th Avenue and see parking on your right. Hazelden entrance is located across the street between Duane Reade Pharmacy and the Oakwood Restaurant.
  • From Connecticut: Take 95 South towards New York City. Merge onto the Cross Bronx Expressway towards the George Washington Bridge/Whitestone Bridge. Take the Henry Hudson Parkway South (Rt 9A) to the West Side Highway. Continue South on West Side Highway to 26th Street. Turn left onto 26th Street. Continue East to 8th Avenue, cross over 8th Avenue and see parking on your right.
  • From Westchester/Yonkers: Take the Henry Hudson Parkway South (Rt 9A) to the West Side Highway. Continue South on West Side Highway to 26th Street. Turn left onto 26th Street. Continue East to 8th Avenue, cross over 8th Avenue and see parking on your right.
  • From New Jersey: Take the Lincoln Tunnel towards 30/31st Street. Turn slight left onto West 30th Street. Turn right onto 9th Ave. Turn left onto 26th Street. Continue East to 8th Avenue, cross over 8th Avenue and see parking on your right. Hazelden entrance is located across the street between Duane Reade Pharmacy and the Oakwood Restaurant.
